Nowadays, there are many providers of patient satisfaction surveys - most are complex and expensive.
www.reportfordoc.com is a start-up created exactly to provide doctors with a simple and accessible tool to pro-actively listen to patients' feedback.

My favorite customer survey is the Net Promoter Score/Ultimate Question Survey. 3 questions, actionable results, all targeted to finding out if the customer would recommend you or not. And it allows them to tell you in their words, not your researcher's, why.
